Purpose

By establishing measures and procedures for handling safety incidents within the SNUH Institute of Convergence Medicine with Innovative Technology (hereinafter the "Institute"), this guideline aims to enable a prompt and systematic response when incidents occur, to minimize injury and loss, and to facilitate activities that prevent recurrence.

Scope of Application

This guideline applies to the Institute's facilities and to all staff and researchers who enter the Institute. Matters not specified in this guideline shall follow the Biomedical Research Institute Safety and Environmental Management Guidelines and the Seoul National University Hospital Safety and Health Management Regulations.

Responsibilities and Authority

Safety management responsibilities at the Institute are assigned as follows.

ICMIT Director

Chief Officer

Serves as the chief officer for safety management within the Institute and appropriately supervises and directs the establishment and implementation of incident management and prevention measures.

Head of the Convergence Medicine Research Support Team

Emergency Response Lead

When a safety incident occurs, serves as the officer overseeing emergency response and management and may directly command and direct response activities.

Incident Reporting Procedure

When an incident occurs, report and respond according to the following six-step procedure.

Initial First Aid & Evacuation

When an incident such as a medical emergency, fire, or injury occurs, the first person to discover it (staff member) carries out, to the extent possible, first aid, evacuation, and alerting activities (activating the alarm).

Emergency Reporting

Depending on the type and scope of the incident, notify the following departments (organizations) first as needed.

  • · Security-related incident: ICMIT Security Office (lobby), ext. 5384

Reporting Up the Chain

When an incident occurs, the staff member reports it to the Head of the Convergence Medicine Research Support Team and reports along the chain to the supervisor / center director / institute director.

Overall Response & Assignment of Personnel

The Head of the Convergence Medicine Research Support Team is responsible for the overall response to reported incidents and may designate personnel within the Institute to carry out related tasks.

Requesting Cooperation from Relevant Departments

The Head of the Convergence Medicine Research Support Team may, when necessary, request cooperation from relevant departments such as the Biomedical Research Institute Division of Laboratory Safety.

Escalation Reporting

The Head of the Convergence Medicine Research Support Team reports to the ICMIT Director and may also report to the Head of the Research Planning & Management Team when necessary.

Incident Reporting System

Incident Investigation, Action, Reporting, and Recording

Incident Investigation and Action

  1. · The department where the incident occurred submits the 'ICMIT Safety Incident Report' (Appendix 1) to the Convergence Medicine Research Support Team within 48 hours of becoming aware of the incident. However, if incident handling is delayed or there are special circumstances and a verbal report has been made, the report submission deadline may be extended.
  2. · Except in special cases, the scene of a safety incident must be preserved in its original state until the investigation into the cause is complete, and must not be altered or damaged without the direction of the Head of the Convergence Medicine Research Support Team.
  3. · When an incident occurs, the Head of the Convergence Medicine Research Support Team oversees the incident response, actions, investigation, and reporting, and may handle these with the cooperation of relevant departments as needed.

Corrective Actions

The Head of the Convergence Medicine Research Support Team and the Institute Director may decide on corrective actions such as the following, taking into account the cause of the incident and subsequent measures, but are not limited to these.

  • · Education and training
  • · Restriction of Institute use
  • · Restriction of Institute access, etc.

Recurrence Prevention Activities

The Convergence Medicine Research Support Team investigates the circumstances and causes of incidents to establish measures for preventing the recurrence of similar incidents, and, through feedback on incident outcomes, continuously improves the safety of the Institute.

Reporting and Recording

The Head of the Convergence Medicine Research Support Team reports on reports, corrective actions, and recurrence-prevention activities to the Institute Director and must maintain and manage the related records.
